Client Rights
At the Freebridge Group, we believe that being informed and supported is an important part of your care. To make things simple and transparent, and in accordance with Texas House Bill 4224, here are instructions for the process of accessing your health records and what to do if you have a concern:

Requesting Your Health Care Records
Your health information belongs to you, and you have the right to review or receive a copy of your records at any time.
How to Request Your Records
Complete a written request including:
– Full Name
– Date of Birth
– Dates of Service
– Specific records or information requested
– You may email this to your therapist or send a secure message through your therapist’s portal.
In order to respect the privacy of each client and maintain HIPAA compliance, we will deliver your records electronically through your secure client portal. We will acknowledge your request promptly and provide the records within 15 business days. Reasonable copying or retrieval fees may apply; we will inform you in advance if any are involved.
Feedback and Concerns
We strive to provide our clients with ethical, respectful, and professional care. If your experience has not met these standards, we would sincerely like to hear your feedback. You can provide this feedback by emailing [email protected] with any concerns you may have.
State Resources and Consumer Protection
We value your trust and take your concerns seriously. If you ever feel that your rights were not respected, you have the option to file a consumer complaint with the appropriate state agency:
